The first problem was survival

Native GLP-1 has a half-life of one to two minutes because DPP-4 removes its first two residues. Exenatide solved this by accident of biology: it is exendin-4 from Gila monster venom, carrying glycine where human GLP-1 has alanine, which DPP-4 cannot process. The first workable GLP-1 agonist was found rather than designed.

Then the problem was duration

Then the constraint changed

Once weekly dosing was achieved, extending duration further stopped being the bottleneck. Effect size became the limit, and the field turned from pharmacokinetics to pharmacology, adding receptors rather than adding hours. Tirzepatide added GIP; retatrutide added glucagon.
