What the 1986 literature reported

Two papers published in Pediatrics in April 1986 examined benzyl alcohol toxicity in very low birth weight infants. Benda and colleagues reported on its impact on neurologic handicaps among survivors; Hiller and colleagues reported on mortality and intraventricular haemorrhage. Together they established a clinical association serious enough to change practice.

Why neonates specifically

Benzyl alcohol is cleared by oxidation to benzoic acid and then conjugation to hippuric acid for excretion. That conjugation pathway is immature in newborns and markedly so in premature infants, so a dose an adult clears readily accumulates. Toxicity here is a consequence of clearance capacity rather than of the molecule being unusually poisonous.

Why the exposure was cumulative and unnoticed

Benzyl alcohol was present as a preservative in flush solutions and diluents used repeatedly through the day in intensive care. No single administration delivered much. The total across many small volumes in a very small patient did, which is exactly the kind of exposure that escapes attention because no individual step looks significant.

Preservative-free formulations became a standard requirement in neonatal care, and preservative-containing products carry warnings against use in that population. The existence of a separate preservative-free product line traces directly back to this episode rather than to a general preference for fewer ingredients.

Bacteriostatic water for injection is sterile water preserved with 0.9% benzyl alcohol. The benzyl alcohol disrupts bacterial cell membranes at low concentration, preventing microbial growth once the vial has been broached. That's why BAC water can be re-entered up to about 28 days after first use — sterile water cannot, because it has no preservative to inhibit contamination.

BAC water is the default for reconstituting research peptides because researchers typically draw from the same vial across multiple sessions. Sterile water is appropriate only for single-use reconstitution or where benzyl alcohol would interfere with a downstream assay (rare, but possible in some cell-culture models sensitive to preservatives).
