Deep dive: why keeping the whole hormone mattered

Every other GHRH analogue in research use is a fragment: the first 29 residues, which carry receptor activation, with stabilising edits layered on. Tesamorelin took the opposite route, retaining all 44 residues and adding a single trans-3-hexenoyl group to the N-terminal tyrosine to block DPP-4 cleavage. The practical consequence is that its pharmacology reads across to native GHRH more directly than any fragment's does, and that its 5,135.9 Da mass puts it among the largest peptides in this catalogue, with the reconstitution care that implies.

Deep dive: the only phase 3 record in the category

Tesamorelin's distinction is not mechanism but evidence. Two placebo-controlled phase 3 programmes, run in HIV-associated lipodystrophy at 2mg daily, measured visceral adipose tissue by CT and produced the 2010 FDA approval as Egrifta. No other GHRH analogue has a comparable file: sermorelin's approval was withdrawn, and the CJC-1295 forms stopped at early-phase studies. The approval is American and population-specific, and it is cited here as the reason human data exists, not as a claim about the research material.

Handling checklist

  • Store the sealed vial at -20°C, protected from light.
  • Reconstitute with bacteriostatic water run down the vial wall; swirl, never shake.
  • A 5,135.9 Da peptide dissolves more slowly than small compounds: allow time.
  • Refrigerate reconstituted solution at 2-8°C and use within 28 days.
  • Aliquot before freezing; avoid freeze-thaw cycles.

Research applications

  • GHRH receptor signalling assays
  • Pituitary growth hormone release models
  • IGF-1 axis response studies
  • Comparative GHRH analogue pharmacology
  • Peptide stability and degradation studies

Common mistakes

Reading Egrifta's US approval as a UK licence
The authorisation is American and population-specific; in the UK tesamorelin is research-use material with no licence.
Treating trial doses as research instructions
2mg daily is the published trial figure, cited as literature; research vials are specified by mass and carry no dose.
Confusing tesamorelin with sermorelin or CJC-1295
Those are 29-residue fragments; tesamorelin is the full 44-residue sequence with a hexenoyl group.
